|Premium/monthly charges||$8.99 for each mobile phone number subscribed.|
|Claim limits||Two claims every consecutive 12 months with a maximum device value of $2,500 each time. If the customer was enrolled in AT&T Protect Insurance for 4 or AT&T Protect Insurance for 1, claims under those programs that are less than 12 months old will be applied to the claim limits of a new certificate of coverage issued by AT&T Mobile Insurance.|
|replacement device||Once the claim is approved, the replacement device is received the next day.* Claims may be resolved with new or AT&T Certified Refurbished equipment that has been previously opened and/or used (may be refurbished or refurbished and may contain original or non-original replacement parts), of the same or other models of the same type and quality. |Device screen repair||As soon as a same-day repair option is available for qualified devices in some areas. The same-day repair option is dependent on claim approval time, parts availability, and technician availability. An Asurion-certified technician performs repairs; This repair includes a 12-month warranty. Repairs may use new or reconditioned parts and may contain original or non-original manufacturer parts, and may void the manufacturer’s warranty. |Covered Incidents||Lost or stolen devices, physical or liquid damage, and non-warranty defects.|
|Cancellation policy||Customer may cancel their optional insurance coverage at any time and receive a prorated refund of their monthly premiums/charges. We may cancel or change the terms by giving you prior written notice as required by law. Any prorated charges or premiums will be refunded to the customer.|
|Insured equipment||Phone: Includes a mobile phone, standard battery, standard battery charger, and SIM card.|
Cordless Home Phone – Includes cordless home phone, power cord, backup battery, phone cord, and SIM card.
Tablet: includes wireless tablet, and if part of the loss, the standard battery charger, and SIM card. For coverage to apply to a particular device, you must own or lease the device and have used (logged voice and data usage) that device on your subscribed wireless line after the initial subscription. Coverage applies to only one device at any given time and the covered device will be the most recently used device on your mobile line at the time of loss.

|Bring your own device||When you activate your device on the AT&T network, you may be eligible to sign up for device protection within 30 days. If the device makes/model is/was previously sold by AT&T, the applicable screen repair and replacement deductible level for that specific make/model applies to all approved claims. For a non-AT&T make/model device, device level C applies. Replacement options vary by deductible level. The device must be in good working order and may be subject to inspection prior to subscription.|
|dispute resolution||In the unlikely event that we are unable to resolve any dispute, including any claim under the AT&T Mobile Insurance program, You shall:|
(1) RESOLVE ANY DISPUTE THROUGH INDIVIDUAL AND BINDING ARBITRATION OR CLAIMS IN SMALL COURT RATHER THAN LAWSUITS IN COURTS OF GENERAL JURISDICTION AND (2) THAT YOU WAIVE YOUR RIGHTS TO A TRIAL BY JURY AND TO PARTICIPATE IN CLASS ACTIONS OR CLASS ARBITRATIONS.

Important AT&T Mobile Insurance Disclosures

AT&T Mobile Insurance is optional insurance coverage that you do not need to purchase in order to purchase services or devices. Replacement enrollment and authorization will be at the sole discretion of Continental Casualty Company, a CNA company; Asurion, the plan administrator; or any other authorized representative of CNA in accordance with the terms of the coverage certificate and applicable law.
Limitations and exclusions
Insurance coverage contains limitations and exclusions. For example, intentional damage, cosmetic damage, and device failure due to defective parts or workmanship are excluded. Complete exclusions and limitations can be found in the enclosed Certificate of Coverage.

If you have provided or will provide your email or other electronic address to AT&T, we may communicate information about the AT&T Mobile Insurance program and legal notices to you through electronic means. If an email is not provided, the information will be sent by postal mail.

Simple claim process
To file a claim quickly and easily, visit phoneclaim.com/att or call 888.562.8662.
- Representatives are available to help Monday through Friday from 8 am to 10 pm ET; Saturday and Sunday from 9 am to 9 pm ET
- The claim must be filed within 60 days of the date of loss.
- If the device is lost or stolen, the customer should contact AT&T Customer Care at 866.MOBILITY to temporarily suspend service to prevent unauthorized use.
- If the device is defective or damaged and a replacement device is provided to the customer, it must be returned with the prepaid shipping label provided with the replacement device. Charges of up to $850 may be added to your wireless bill if a defective or damaged device is not returned.
- Once the claim is approved, the customer can receive their replacement device the next day. Deliveries to Alaska, Hawaii, Puerto Rico, and the US Virgin Islands cannot be shipped for overnight delivery.
The enclosed Certificate of Coverage may provide a Duplicate of coverage already provided by the consumer’s personal auto insurance policy, homeowner’s insurance policy, renters insurance policy, personal liability insurance policy, or another source of coverage. This insurance will be considered primary over any other insurance you have. Unless licensed, AT&T partners are not qualified or authorized to evaluate the adequacy of Your existing insurance coverage. Questions about this plan should be directed to a licensed agent of the CNA, Asurion Protection Services, LLC.
The enclosed Certificate of Coverage is the entire agreement between you and CNA. Customers should refer to the Certificate of Coverage for full terms and conditions of coverage provided (including exceptions set forth in Section X. CHANGES IN STATUS). In the unlikely event that we are unable to resolve any dispute, including any claim under the attached AT&T Mobile Insurance Certificate, You should note that THE FOLLOWING TERMS AND CONDITIONS CONTAIN A BINDING ARBITRATION PROVISION THAT REQUIRES SUBMISSION OF ALL DISPUTES (EXCEPT WHERE EXPRESS WAIVERS ARE PROVIDED) FOR FINAL AND BINDING ARBITRATION IN ACCORDANCE WITH THE PROVISIONS SET FORTH IN SECTION VIII. G. OF THE INCLUDED TERMS AND CONDITIONS.
IMPORTANT: Any person who knowingly and with intent to injure, defraud, or mislead an insurer makes a statement of claim or application that contains false, incomplete, or misleading information is guilty of insurance fraud. In Florida, such conduct is a third-degree felony. This notice is not effective in Oregon.
